MEDICAL CARD APPLICATION

If you feel you are entitled to a Medical Card or Doctor Visit Card or to check if you are eligible for a card go the the HSE website. This is the quickest way to apply; you should receive an answer from the within days. Click the following link apply and for more information

OVER 70 GP SCHEME

If you are aged 70 or over you are eligible to apply for an Over 70 Doctor Visit Card. The four GPs in this Practice are registered for the scheme. To register for this scheme please click on the following link. If you do not register, you will not be entitled to free GP care.

UNDER 6 GP SCHEME

The four GPs in this Practice are signed up for the under 6 scheme. If you are eligible for this scheme please ensure you register your children. The scheme will commence on 1st July 2015, if your children are not registered they will not be entitled to free GP care. Click the following link to register

MEDICAL CARD FOR DOMICILIARY CARE ALLOWANCE (DCA)

Certificates

Social welfare certs can be requested by email, they will be sent electronically by the doctor to  dept of social welfare, Work certs can be ordered and paid for online and email to the patient once approved by the GP.

Order repeat prescription online, script will be sent to your pharmacy of choice electronically within 24hours.
